Homecare Caregiver/CNA

We are looking for individuals with flexibility to work with our homecare agency. Must be dependable, caring, long and short shifts available as little as two hours. The in-home caregiver provides compassionate care to disabled individuals in the home. The candidate must have experience caring for others, know body mechanics and patient safety measures to be accepted for this position.

Responsibilities:

  • Assists patients with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan.
  • Allows and encourages patients to do as much of their own care as possible.
  • Prepares and serves meals to patients and may assist in preparing meals following preplanned menus.
  • Encourages patients to eat a proper diet and take adequate fluids while respecting the patient’s right to free choice and records and reports changes in patient’s eating habits to the supervisor.
  • Initiates and participates in leisure activities provided for patients as described in the activity calendar and encourages patients to socialize and participate in planned activities or programs.
  • Maintains patients’ records daily in a timely manner and in accordance with company policy and procedures; documents medication distribution as applicable, leisure activities, incidents, and observations; reports any changes in patient’s physical condition and/or behavior, and visitors; observes and reports the health and emotional condition of the patient, and promptly reports all changes to the supervisor.
  • Assists patients with medication reminders as defined in medication procedure; assists or supervises patients who self-administer the medication; and exhibits an understanding of and follows medication policies and procedures.
  • Maintains a clean, safe, and orderly environment for the patients; performs general housekeeping; and follows cleaning schedules for patient laundry, bedrooms, dining area, living space, bathrooms, kitchen, and other areas.
  • Follows proper procedures in emergency situations and responds promptly and positively to patient requests for assistance.
  • Knows the location of the patient and notifies the supervisor immediately if unable to locate a patient.
  • Encourages teamwork through cooperative interactions with co-workers.
  • Some driving may be required for appointments. You will be reimbursed for any miles driven during your shift.

  • Education, Experience, and Licensing Requirements:
  • High School Diploma or GED
  • CPR/FA, 1 & 2 (within 6-12 months of hire)
  • Any state-mandated certification
  • Minimum 1+ year(s) of professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors
  • Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years
  • Must be over 21 years old

Registered Nurse/RN

Responsibilities:

  • Monitoring patient’s condition and assessing their needs to provide the best possible care and advice.
  • Observing and interpreting patient’s symptoms and communicating them to physicians.
  • Collaborating with physicians and nurses to devise individualized care plans for patients.
  • Observe and record patient behavior
  • Perform physical examinations and diagnostic tests
  • Collect patient health history
  • Counsel and educate patients and their families on treatment plans
  • Administer medication, change wound dressings and care for other treatment options
  • Read and interpret patient information, making treatment decisions when appropriate
  • Consult with supervisors and doctors to decide on the best treatment plan for patients
  • Research ways to improve healthcare processes and improve patient outcomes.
  • Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts.
  • Adjust and administer patient’s medication and provide treatments according to physician’s orders.
  • Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
  • Provide instant medical care in emergencies
  • Assist surgeons during operations
  • Supervise and train LPNs and nursing assistants
  • Foster a supportive and compassionate environment to care for patients and their families
  • Expand knowledge and capabilities by attending educational workshops, conferences etc.
  • Direct and supervise nurses, nurse assistants and nurse aides.

Qualifications/Skills:

  • Proven experience as a registered nurse
  • Excellent knowledge of nursing care methods and procedures

  • In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times

LPN

LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing diverse basic care to those who need it while remaining patient and calm. It is a demanding job but also very rewarding. Responsibilities

Responsibilities:

  • Record a patient’s medical history accurately
  • Take and record measurements of blood pressure, temperature, heart rate etc.
  • Observe patients under treatment to identify progress, side-effects of medications etc.
  • Monitor patients’ condition including fluid intake and output and compose patient charts
  • Assist ailing patients in daily necessary activities such as eating, bathing etc.
  • Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
  • Collect samples for lab testing as assigned
  • Provide emotional and psychological support when needed
  • Communicate with patients’ family or friends to provide advice, comfort and release instructions

Qualifications/Skills:

  • Proven experience as LPN or similar role
  • Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
  • Familiarity with hospital health, safety and sanitation standards and procedures
  • Understanding of confidentiality obligations and nursing best practices
  • Excellent knowledge of medical and hospital terminology
  • Good knowledge of MS Office and data entry
  • Perfect physical condition and stamina
  • Excellent communication and interpersonal skills
  • Compassionate and able to handle stresS
  • Successful completion of LPN license program is a must
